GUIDELINES FOR THE INTAKE ROLE

1. The worker must get identifying information. This may include name, address, telephone, composition of family, marital status, age, ethnicity, Social Security number, place of birth, income, source of income, history of psychiatric treatment, bank accounts, where children are attending school, financial assets, ... and the problem the person is concerned about.

2. The worker must record the required information in the agency's recording
system.

3. The worker must try to understand how the client views the problem (which
may be very different from how the worker views the problem, or how the
agency views the problem].

4. The worker must know how the agency views the problem.

5. The worker must convey to the client enough information about the agency's view so that the client can take the steps she or he wants to take.

6. The worker must offer appropriate emotional support while providing the
information, so the client feels valued, even if the worker cannot offer what
the client would like.

7. The worker must help the client decide what to do by posing alternatives in
a way that presents choices clearly and fairly.
